Instructions

This form should be completed within the first week of classes.
The student must complete Section A — Student Information.
The form must then be submitted to the appropriate former Lab Instructor/Lecturer for completion of Section B — Lecturer Information.
Submission of this form does not automatically mean that a laboratory exemption has been approved.

Important: Your previous laboratory performance must be reviewed before a recommendation for exemption can be made.
