Complete this form if you are requesting to change from one program of study to another. Fields marked * are required.

Before you begin

This form is only for students changing from one program to another, for example Agriculture to Marine Science, or changing to a certificate program.
A change of program may affect the duration of your studies. Students should seek advice from their funding body regarding any funding implications.
A change of program may result in the loss of credits.
There is a $40.00 Change of Program charge.
Payments may be made through the BAMSI online payment system, or the student's account may be automatically billed.
